Summary

A scar is a mark left after healing of an accidental or surgical wound, but it can also be due to a more or less severe form of acne that has left traces.

To better understand the scarring process and how to diminish their appearance, we will ask ourselves:

  1. How does a scar evolve?
  2. What factors can slow healing?

1.How does a scar evolve?

Several physiological actors intervene to allow the reconstruction of the epidermis resulting in healing.

First of all, a blood clot forms to stop the flow and temporarily close the wound to prevent the infiltration of viruses or bacteria.

Afterwards, the collagen fibers fill the wound thus reforming the epidermis.

Finally, the healing phase begins. The appearance of the scar evolves for 1 year to 1 year and a half while the tissues remodel themselves little by little. The inflammation gradually decreases over the months, after reaching a peak between the 4th and 6th week. The skin then regains some elasticity and a paler color, until it turns into a whitish line or plaque.

A scar will remain more fragile than normal skin, it contains less elastic collagen fibers, less well irrigated and less hydrated.

Surgical wounds with smooth, tight edges heal quickly and easily. The scar is usually thin and almost invisible.

Wounds with irregular edges take much longer to heal. The scar will be larger and more visible.

In black and Asian skin types, healing is more difficult, because the skin reacts when it is attacked by producing melanin, which sometimes leaves scars pigmented.


2. What factors can slow healing?

Several factors can interfere with proper healing:

  • Sun exposure risk of pigmenting the scar with a brown, blotchy discoloration that will not go away. As long as the scar is red or pink, it will be very sensitive to UV rays, which is why it will have to be covered with a bandage or plaster if you plan to expose yourself. Otherwise, sun protection is essential (SPF30).
  • The tobacco : Smokers heal less well because of the vascular effects of tobacco, skin cells are less nourished and oxygenated.
  • The stress may interfere with healing due to cortisol secreted, which has vasoconstrictor effects.
  • extreme temperatures : Recent scars from 6 months to 1 year should avoid extreme temperatures such as sauna, hammam or intense cold. This has a negative impact on the newly formed tissue very sensitive.
  • Medication : Corticosteroids and non-steroidal anti-inflammatory drugs delay healing by their anti-inflammatory action and inhibit the synthesis of collagen. Chemotherapies, local anesthetics and immunosuppressive treatments have also deleterious effects.
  • Some diseases such as diabetes, chronic hyperglycemia or diseases affecting blood vessels, venous insufficiency, or arteritis delays the healing process. The consequences will be the same for patients with AIDS, cancer and kidney failure.
  • Unsuitable diet: A diet that is too rich and too sweet can accelerate its degradation, resulting in permanent stiffening of the collagen fibers and therefore greater difficulty in healing 
  • Consuming foods rich in Vitamin C is essential for the formation of collagen and will prevent infections. Zinc, found in certain foods (oysters, shellfish, eggs, etc.) helps strengthen the immune system and activates wound healing.
